Hier beschrijven we een casus-serie van drie patiënten met acute promyelocytenleukemie (APL) en beenmergnecrose. Beenmergnecrose is een zeldzame maar ernstige complicatie bij patiënten behandeld met arseentrioxide (ATO) en ‘all-trans retinoid acid’ (ATRA). Er is weinig bekend over de incidentie, te verrichten diagnostiek of behandeling van beenmergnecrose. Naar aanleiding van deze drie casussen doen we een voorstel om 1) bij invaliderende pijnklachten en een sterk verhoogd LDH een MRI te verrichten en indien deze inconclusief is een beenmergaspiraat te overwegen, en 2) bij vastgestelde beenmergnecrose te overwegen ATO/ATRA tijdelijk te onderbreken.
