Elke medische behandeling begint met het stellen van de juiste diagnose. Deze diagnose ligt echter niet altijd meteen voor de hand. Zoals bij de volgende casus:
Een 59-jarige man werd overgeplaatst naar ons academisch centrum vanwege een onbegrepen beeld van snel progressief gegeneraliseerd oedeem en lymfadenopathie. Hij had zich aanvankelijk gepresenteerd met koorts en oedeem met in het laboratoriumonderzoek trombocytopenie (43 × 109/l), anemie (Hb 6,9 mmol/l) en nierfunctiestoornis (eGFR 26 ml/min). Er was verder sprake van ascites, pleuraen pericardvocht en het gewicht was in enkele weken 20 kilogram toegenomen.
