Angiosarcomen zijn zeldzame solide maligniteiten met een slechte prognose en ontstaan zeer zelden primair in de milt. In deze casus presenteren wij het diagnostisch traject van een patiënte die als eerste uiting van haar angiosarcoom een spontane miltruptuur kreeg. Hierop volgde ontwikkeling van een leuko-erytroblastair bloedbeeld en dreigend leverfalen, secundair bij beenmerg- en levermetastasering. Nadien wordt ingegaan op de differentiaaldiagnose van spontane miltbloedingen en angiosarcomen.
